Patient Forms

• Once you have scheduled an appointment, you will be sent a “New Patient Packet” via email prior to your visit.  Other forms may also be sent to you depending on what your primary complaint is.
• Please complete the forms that you received and return them AT LEAST 72 hours prior to your visit.  It is also helpful to provide any recent lab work that has been performed.

Online Dispensary

• Fullscript and Wellevate are online dispensaries utilized allowing for professional grade products to be shipped directly to you
• Access to the online dispensary will be provided to patients
• All products will be at a discounted rate for patients

Note: Please be advised that supplements are an unregulated industry.  If you choose not to purchase your supplements from my online dispensary, please get them from a trusted source (try to avoid sites like eBay, Amazon, etc. to get your supplements).  Most supplements purchased via the online dispensary are higher quality. They have been tested by a third party company to ensure that each supplement is what the label says it is, and majority are free of common allergens and additives.
